By submitting this form, I agree that PnB Athletics, its coaches, owner, and all of their officers, employees, volunteers, and agents are not responsible or liable for any injuries, accidents, or other incidents that may occur as a result of participation in practices, games, training sessions, court / space rental or related activities. I acknowledge that I am voluntarily allowing my child to participate and that I assume all risks associated with such participation. I certify that my child is physically fit to participate and that I am responsible for any medical expenses that may arise from any injury or illness.
